Individual Participant Statements

We will prepare a quarterly statement for each participant which shows his or her account balance at the current allocation date. This statement reconciles the account balance from the prior allocation balance and shows the current vested portion of the account. This statement can serve as a useful communication tool so the employees can become more aware and appreciative of the plan.

We will prepare a statement showing the vested account balance for each participant who terminates.
 

Annual Reporting and Disclosure

We prepare the following forms and reports as part of our annual service for the plan:
Form 5500 series and all supplemental schedules.
Summary Annual Report (SAR) to be distributed to all plan participants.
