Annual Allocation ReportThis report presents the results of the allocation of the plan and includes the following:
- Employer desired allocation formulas.
- Statement of asset transactions during the plan year.
- Listing of all participants with a reconciliation of account balances from one year to the next. This reconciliation of stock and cash includes allocation of employer contributions, forfeitures, and fund earnings to each participant's account. This listing also provides the vested percentages for each participant.
- Separate lists for participants who have separated from service with the Company, but who have not received their plan benefits.
- Summary of the principal provisions of the plan.
Individual Participant StatementsWe prepare an annual statement for each participant which shows his or her account balance at the current year-end. This statement reconciles the account balance from the prior year's balance and shows the current vested portion of the account. We show stock shares separately from other investment accounts. This statement can serve as a useful communication tool so that the employees can become more aware of and better appreciate the plan.
Annual Reporting and DisclosureWe prepare the following forms and reports as part of our annual service for the plan:
- Form 5500 series and all supplemental schedules.
- Summary Annual Report (SAR) to be distributed to all plan participants.
Compliance TestingWe monitor all limitations and tests, such as:
- Code Section 415(c) limits
- Compensation limits and testing under Code Sections 401(a)17 and 414(s)
- Deductibility limitations under Code Section 404
- Coverage requirements under Code Section 410(b)
- Non-discrimination requirements under Code Section 401(a)(4)
- Top-heavy analysis under Code Section 416.
- Code Section 1042 Transactions.
- Others as they may apply.
